Preparing For Your Visit

In preparation for your appointment with us, please take note of your check in time. It is imperative that that you adhere to your check in time to allow us time to process your paperwork, insurance, and medical history before your appointment.

Be sure you have your insurance card and drivers license with you and remember that payment of co-pays and deductibles are expected at the time of service.

Children and guests are not allowed to go back with you to your appointment and children are not permitted to remain in the waiting room alone.

Please bring a list of all medications you are currently taking.

If you have any device or implants, please bring your implant card with you.

Remove all jewelry and piercings prior to your appointment.
